What you’ll need in order to apply:
- Review the full set of Laws and Regulations regarding the practice of Athletic Trainers in the state of Virginia
- Carefully read through the application instructions for details
- Request certification from the Board of Certification for the Athletic Trainers (www.bocatc.org) to be sent directly to the Virginia Board of Medicine
- Complete the online application

Reinstatement of Athletic Trainer License
You have indicated that you would like to Reinstate your lapsed Athletic Trainer license. Please note that you may reinstate a license only after it has been expired for two years.
This Reinstatement application is a Word document that will need to be printed, filled out and sent to:

What you'll need to apply:
- Review the full set of Laws and Regulations governing the practice of Athletic Training in the state of Virginia
- Carefully read through the Reinstatement application instructions for complete details.
- Verification of Athletic Trainer licenses from all jurisdictions within the United States, its territories and possessions or Canada
- NPDB Self Query – complete the online “Place a Self-Query” Order form.
- BOC Certification
- Copies of any documentation supporting any name change since your initial licensure in Virginia.
- If you answer “yes” to any question 6-18, provide documentation to the Board from your attorney or you may provide a narrative explaining your answer.
- Completed Reinstatement application and check or money order made payable to the "Treasurer of Virginia" for the amount indicated on the reinstatement application.
