Once your certificate has been processed, you will be sent an electronic copy of your CDS Registration Certificate to the email address listed on your CDS application. In addition, you can visit the CDS search page and print your CDS registration information page to use as verification of your CDS registration.

All CDS applications for a new registration and renewal registration are required to be electronically submitted.
OCSA will no longer be accepting paper applications unless it falls into one of these categories:
1.) Address Change 2.) Name Change 3.) Changes of Ownership 4.) Duplicates

PDMP Mandate Effective February 15, 2018
All MD licensed healthcare providers must register with the Prescription Drug Monitoring Program (PDMP) prior to obtaining a new or renewal of CDS Registration. To register with PDMP, go to CRISP website at https://crisphealth.org/ Submit to OCSA the PDMP email confirmation that includes the confirmation code number.
