CDS Application

Once your certificate has be​en processed, you will be sent an electronic copy of your CDS Registration Certificate to the email address listed on your CDS application. In addition, you can visit the CDS search page​ and print your CDS registration information page to use as verification of your CDS registration.​

Effective December 31, 2020

All CDS applications for a new registration and renewal registration are required to be electronically submitted.

OCSA will no longer be accepting paper applications unless it falls into one of these categories:

1.) Address Change 2.) Name Change 3.) Changes of Ownership 4.) Duplicates ​

PDMP Mandate Effective February 15, 2018

All MD licensed healthcare providers must register with the Prescription Drug Monitoring Program (PDMP) prior to obtaining a new or renewal of CDS Registration. To register with PDMP, go to CRISP website at https://crisphealth.org/ Submit to OCSA the PDMP email confirmation that includes the confirmation code number.
